This new model underscores federal policies around health equity, health-related social needs, and alternative payment models for primary care providers. In this webinar series, PCDC will break down this comprehensive and complex program into simple components. The first webinar is specific to federally qualified health centers’ needs, the second is tailored to independent practices, and the third is to discuss any unanswered questions.
