Population Health Summit 2017: “Five-Year Retrospective: How Far We Have Come, and What Lies Ahead”

The New York Health Foundation, The New York Academy of Medicine, and the Department of Population Health, NYU Langone Health are sponsoring the fifth annual Population Health Summit, “Five-Year Retrospective: How Far We Have Come, and What Lies Ahead,” on December 11, 2017.

Nonmedical determinants disproportionately affect population health. The evidence is clear that where people live, eat, work, and play directly affects their health outcomes. In response, the health care and social service sectors have coordinated a wide range of activities to bridge health care and population health activities.

Started in 2013, the annual Population Health Summit seeks to deepen and extend the understanding of ongoing efforts in New York State and nationally that demonstrate effective collaboration between health care and other sectors to advance population health. A five-year retrospective on the state of population health will examine how far we have come and the field’s readiness to address what lies ahead. With a new Administration and emerging public health crises such as the opioid epidemic, the time is ripe to reflect and plan ahead.

The goals of the Population Health Summit are:

1. Feature national, State, and city leaders and case studies that highlight progress, lessons learned, and opportunities to replicate population health efforts in New York State;
2. Connect statewide and citywide efforts to help advance collaborations between health care systems and other sectors to advance population health; and
3. Facilitate networking between health care providers, insurers, business leaders, policymakers, public health and community leaders, and researchers.
