Two federally qualified health centers, Anthony L. Jordan Health Center (Jordan Health) and Westside Health Services (Westside) provide health care to Rochester’s low-income and underserved population.
By the end of 2011—after years of financial difficulties—Westside was on the brink of closing its two sites. Losing Westside would be detrimental to its already underserved communities: about 20,000 patients with high rates of chronic illness, mental health disorders, and substance use issues would lose access to primary care. After substantial consultations and feasibility planning, Jordan developed a plan to take over Westside and operate its two sites under Jordan’s corporate umbrella.
In 2011, NYHealth awarded Jordan Health a grant to support the acquisition of Westside’s sites.
Outcomes and Lessons Learned
- Transitioned and kept open Westside’s two locations as a level-three patient-centered medical home offering safety-net services to the community;
- Integrated Westside staff members and patients, billing services, and information technology systems into one unified administrative structure under Jordan Health; and
- Achieved and maintained financial viability at both acquired sites.
