SEE GRANT OUTCOMESPeople with diabetes are more likely to have multiple chronic conditions and are in need of self-management programs that will engage them to better manage their care and medication regimen.
The National Council on Aging (NCOA) established the Self-Care Management Alliance (the Alliance) to bring to scale proven self-management programs (including spreading the use of the online Chronic Disease Self-Management Program, or CDSMP), increase community capacity to offer services to help people manage their conditions, and create financing mechanisms to support self-management programs.
Through the Alliance, NCOA achieved the following:
- Synthesized and disseminated the evidence regarding self-care management programs;
- Provided input into the Centers for Medicare and Medicaid Innovations regarding opportunities to employ self-management principles in guidance and technical advice on accountable care organizations;
- Developed strategies to expand and sustain the CDSMP in community-based organizations and online; and
- Educated policymakers about opportunities to expand access and financing for evidence-based self-management programs.
