Care management, which includes facilitating access, coordination, and communication for patients’ care plans, is an essential element of the patient-centered medical home model, and its integration into practice is a core strategy for primary care transformation.
Because care management requires a fundamental shift in health care organization and delivery, it is essential to establish a statewide consensus on its definitions, functions, and requirements. Additionally, sustainable implementation of care management requires that health care practices determine the most efficient way to deliver these skills. In New York State, Medicaid health homes provide an opportunity to support better care management, improve health outcomes, and reduce health care costs.
In 2013, NYHealth awarded Joslyn Levy & Associates (JLA) a grant to issue a report on emerging and evolving care management strategies in New York State.
