The New York Diabetes Coalition (NYDC) is a volunteer collaboration of more than 70 organizations across New York State, including health plans, professional medical societies, and health-focused and community-based organizations dedicated to improving care for New Yorkers with diabetes. NYDC will work with the New York Health Foundation’s Diabetes Campaign to assess opportunities for and barriers to improving health outcomes for patients with diabetes in primary care practices.
As part of this initiative, the Diabetes Campaign staff also will convene three regional stakeholder meetings to identify barriers to and opportunities for achieving high-quality standards of care; determine where assistance is needed to improve health outcomes; and to identify specific institutional, local, and regional resources that can be leveraged to support widespread improvements.
