DOWNLOADThis Rockefeller Institute brief reviews eight patient-centered medical homes (PCMH) underway in New York State. PCMHs have emerged as a team-based, primary care model that focuses on continuous and coordinated care.
Core elements of this model include an ongoing relationship with the primary care doctor, increased access to care and services, and a value-based payment system. PCMHs are expected to result in comprehensive care and improved health outcomes for the patient, while reducing care costs.
The brief describes what was learned about New York’s PCMH pilots and examines some of the policy implications of PCMHs. One of the PCMHs discussed in the findings, Excellus and Preferred Care/MVP’s Rochester Medical Home Initiative, is an NYHealth grantee.
