Tools and Guidelines for Planning Effective Project Evaluations

This set of guidelines, developed by the Center for Health Care Strategies for NYHealth, is designed to help grant applicants think about and formulate a program evaluation as part of an NYHealth grant proposal.

The guidelines offer a range of information, tools, and resources to assist applicants with planning, designing, and implementing an evaluation plan that can improve the outcomes and impact of a grant project.
